Transition Home Program What is the Transition Home Program? Spending a prolonged period of time in a hospital or rehabilitation facility may leave you concerned about how you’ll manage once you return home. This is especially the case if you have to continue with medical treatments, manage medications, or change your lifestyle to improve your health. Our two-day Transitional Home Program assists you in completing this transition as smoothly and comfortably as possible. Contact Us to Learn More Day One: Getting Settled In Our dedicated caregivers will make sure your transition is safe and full of love, light, and laughter. Your nurse will: Meet with you and supervise the hospital discharge process with planners and Medicare providers. Review the hospital’s discharge information with you. Accompany you back to your home. Pick up prescription medications, medical equipment, and groceries. Prepare a healthy meal. Clean and tidy up your home. Day Two: Coordinate Your Ongoing Care Now that you’re settled in, your caregiver will: Schedule medical appointments. Coordinate Medicare agency visits. Coordinate skilled nurse and/or therapist visits (for physical, occupational, or speech therapy). Monitor your progress. Drive you to follow-up medical appointments.
